Children’s community home care teams

Our children's home care team work with children, families, carers, nursery and school staff to enable children to attain and maintain their optimum level of health within their home and school environment.

The children's home care team supports and helps parents and carers to care for their sick or injured child at home, reducing the need to travel to and from hospital and ensuring as little disruption as possible to the routine of our young patients and their families.

We work closely with families, social care, education, therapists, GPs, consultants and voluntary agencies to ensure that all required services and equipment are in place across all aspects of the child's life prior to discharge.
